To ensure that your issue is resolved in a timely fashion, please gather as much of the following information as possible:

• Date and time of the error
• Error code number and message
• Your operating system (e.g., Windows 7), Internet browser (e.g., Firefox 20), and version of affected computers
• WVEIS (SSID) number of the affected student(s) - For security reasons, do not include names.
• Session ID
• Grade, subject, test, and question number (if applicable) - For test security reasons, do not describe the content of the question.
• If you are reporting for another person, please provide the other person’s contact information if possible.
